This week, our care coordinators came together for their regular planning workshop. Care coordinators work at the frontline of our services and play an essential role in supporting Clinicians to deliver patient services. Over the last six months, the team has supported the PCN in delivering vital campaigns, including Enhanced Access, Spring and Winter Vaccination
